To better optimize your time with our veterinary team, we are asking clients to please fill out our history questionnaire and return it 48 hours prior to your cat's visit. This will allow our veterinary team to review your pet's needs and your questions prior to your visit and allow for a more complete and cat friendly examination and discussion. Do you have any initial concerns you would like to discuss? If so, please describe them below and give us an idea of how long these changes have been a concern. Have you noted any vomiting (including hairballs), diarrhea, coughing, or sneezing in your cat? If so, how much? How frequent is the episode? How long has it been going on? 3. How is your cat's appetite? What do they like to eat (please note brand and flavors)? How often do they eat and how much at each sitting or how much overall during the day? Have there been any changes to your cat's appetite? What kind of treats to they eat? 4. How is your cat's water consumption? How frequently do they visit the water bowl? Do they drink from a fountain or a bowl? Has there been any change in your cat's thirst? 5. How are your cat's litter box habits? How often do they urinate? Have you noted any changes in frequency or amount of urination? How often does your cat defecate? Have you noted any changes in frequency or consistency of your cat's stool? How often is the litter box scooped? How many litter boxes are in your home? 6. Is your pet on flea, tick, heartworm, or intestinal parasite preventative? If so which one? 9. Does your cat take any medication or supplements? If so please tell us which ones, how much, and how often. 10. Is there any history prior to adoption or previous diagnoses that you would like us to be aware of or you would like to make note of? 11. Gabapentin used in appropriate doses can help take facilitate a stress-free exam for a fearful or stressed cat who visits the veterinary clinic. It can provide temporary anxiety relief for your cat while he or she is being examined by a strange person in a strange place. It does not completely sedate your cat, but rather allows them to have relaxation while still being aware of their surroundings. If your cat has been seen by us before (within the last year) and is not taking gabapentin for veterinary visits, but you are interested in helping your cat feel as calm as possible during visits, please call our front office to request the mediation. If you're a new patient or have not been seen within the last year, we may be able to recommend this medication after your cat's initial visit. If your cat is already taking Gabapentin to help facilitate stress free exams, please note how much your kitty receives prior to examination and please tell our veterinary team at the time of check in if your cat received this medication and when.
